In the form provided by Distributor (in Word format), one (1) complete, accurate and typewritten statement of: a) all screen and advertising credit obligations/restrictions, b) all name and likeness obligations/restrictions, c) all photo approvals, and (d) any other third party obligations, restrictions and approval rights, for all companies, cast and crew members who have credit placement stipulated in their contracts  and accorded credit in the billing block and main credits, shall be listed in the order of how those entities and individuals appear in the billing block and main credits. If there are no screen credit obligations, paid advertising and credit obligations and/or name and likeness restrictions, a signed written statement upon which Distributor may rely must be provided by Company.

Restrictions Log (Series)

For series, please provide the Talent Restrictions and Approvals Chart as noted above, or if already in place, a restrictions log that lists ALL circumstances in which distribution rights could not be obtained and any ad/promo restrictions.
